Acknowledgment of Camp Eligibility, Policies & Risks

Please review the statements below and acknowledge your understanding of the terms, policies, and risks associated with your student's participation in Spotlighters Camp.

I, , acknowledge that my camper(s) will be participating in Spotlighters Show Choir Camp, organized by Upstairs Talent Show Productions LLC (d/b/a Spotlighters Show Choir Camp), taking place at Deshler High School from July 7th to July 12th, 2025.

I understand that camp activities include, but are not limited to, music classes, choreography sessions, team-building games, and final performances on Friday, July 11th, and Saturday, July 12th, 2025.

Essential Eligibility Criteria & Participation Requirements

I confirm that my child meets the Essential Eligibility Criteria for participation, including:

Entering Grades 1-12 in the 2025-2026 school year.
Independently managing personal hygiene and self-care needs.
Actively participating in singing, dancing, and learning choreographed movement.
Following verbal or visual directions, engaging appropriately in group activities, and respecting camp staff and peers.
Managing transitions and emotions appropriately and maintaining a safe and positive camp environment.

Camp Policies & Payment Terms

I acknowledge and agree that:

Full payment is required at registration to secure my child's spot by June 1st.
If I withdraw my child before May 1st, 2025, I will receive a full refund minus a $50 processing fee per camper. No refunds will be issued after May 1st, and tuition is non-transferable.
Absences will result in my child being unable to participate in the final performances on July 11th and 12th, 2025.

Safety, Medical Information & Medication Policy

I understand that:

My child’s medical information, including allergies and required medications, must be disclosed before camp for safety protocols.
Staff cannot administer any medication. If my child requires emergency medications (such as an EpiPen or inhaler), they must self-administer, and I must inform camp staff prior to attendance.
In case of an emergency, 9-1-1 will be called, and my child may be transported to Helen Keller Hospital or North Alabama Medical Center for treatment if necessary.

Behavioral Expectations & Substance Policy

I acknowledge that my child must:

Follow the camp’s behavioral expectations, treating others with respect.
Refrain from possessing or using alcohol, tobacco, illegal drugs, or non-prescribed medications. Necessary prescription medications must be disclosed in advance for safety compliance.
Understand that any damage to property (accidental or intentional) may require repair or reimbursement.
Avoid foul language or obscene gestures, which will not be tolerated.

Liability Waiver & Photo Release

I understand that:

Upstairs Talent Show Productions LLC is not responsible for any areas of Deshler High School outside of the Classrooms, Lunchroom, Auditorium/Theatre, and Designated Green Spaces during camp hours.
Despite reasonable safety measures, participating in choreography and performances involves inherent risks. At least one staff member trained in first aid and CPR will be present.
By signing below, I consent to photographs and videos of my child being used for promotional purposes by Upstairs Talent Show Productions LLC.
